How to Order
Please fill out the following information to reorder contact lenses. You must be a current patient at our office and have a current and valid prescription. We will order the lenses and notify you when they are ready to be picked up. Payment is due upon receiving the lenses. Failure to pick up contacts within two weeks may result in a restocking charge to your account.
